很多人知道威而钢(Viagra) 是治疗男性阴茎勃起功能失调(俗称阳萎)很有效的口服药剂。在民国八十七年三月二十七日美国食品药物管理局(FDA)核准第一种治疗阳萎的口服药正式上市之后，有关于阳萎的话题，即不断地在世界各地的广播、网路、电视及报章杂志等媒体大谈特谈，「威而钢」大名远播，赚进了大把的钞票，威而钢的震撼至今仍然不断，但余威之下是否会有副作用，是想要尝试的人一直关心的事。 辉瑞药厂自1991年开始研发sildenafil，原先是想把它当成缓和心绞痛的药物，但施行人体试验的结果不理想准备放弃，却意外地发现，部份试用的男性答称服用后可以改善勃起功能。因此研究小组开始根据现有事实研判，进而转移目标发展成治疗阳萎的药「威而钢」。 当男性受到性刺激时，性冲动引起的电波传到阴茎海绵体内，其内的nonadrenergic noncholinergic(NANC)的神经元及海绵体肌肉的内皮细胞(endothelial cells)就会释放出氧化氮(NO)，使细胞内的催化酵素—(Guanylate cyclase)产生作用，将(GTP)转化为(cGMP)，这种化学产物会让阴茎海绵体的肌肉松弛，血管扩张使血液大量涌入引起勃起反应。男性只要持续受到性刺激，便会不断地分泌出cGMP，但同时，阴茎海绵体细胞会分泌另一种酵素5(PDE 5)，这种酵素会摧毁cGMP使它转变成为不具活动性的(GMP) ，使阴茎勃起消退，结果是消长维持均衡，使阴茎保持勃起。威而钢的功能就是阻断PDE 5的作用，使阳萎的病人服用后，在性刺激下，使海绵体细胞分泌出来的cGMP减退变慢，助长cGMP的作用，使阴茎能达到勃起的疗效。因此，它不是催情剂(俗称春药)。根据美国FDA的报告中指出，从三月底至七月间，医师已经开出360多万个威而钢的处方，目前世界各地，包括墨西哥、巴西、欧洲地区及澳洲等国都已陆续正式核准上市，马来西亚及新加坡也将于最近通过公开使用，本国的病患也期盼它快速通过卫生署的核可。但它的效果到底如何？ 全世界共有4274位阳萎病人(年龄介于18-87岁，平均患阳萎时间为5年)，接受随意抽样平行双盲的封闭式临床试验，分别给予病人使用威而钢或外型与威而钢类似但分办不出真伪，其实不含药成份的安慰剂，试验过程中病人及医师都不知道病人服用的是真或假药丸，等实验结束才由提供试验的辉瑞药厂总部开封验证及统计结果，本项试验的结果分析有2722位病人服用威而钢，有1552位病人服用安慰剂，持续服用6个月，而后总共有2199人采开放式继续服用威而钢达一年之久。这些人包括过去曾试用威而钢者1430人及安慰剂者769人，发现封闭式服用固定剂量的威而钢及安慰剂的病人性功明显改善的分别是82%及24%，两者之间有相当的差异，而在服药过程中，因为有副作用而中止服药的，两者则没有差别，其比例分别是威而钢占2.5%，安慰剂占2.3%。中止治疗主要原因是头痛(占1.1%)，脸潮红及恶心(各占0.4%)，服用安慰剂而中止治疗的原因，大部份也是因为头痛(占0.4%)。在采开放式持续服用威而钢达一年之2199人中，结果发现有88%的病人明显有效改善他们的性勃起。
